Parishioners, Committee Contact Persons, Staff and Volunteers are occasionally asked to participate in church publicity, publications and/or public relations activities.  To protect your privacy and ensure your agreement for participation, St. Jude Catholic Church asks that you sign this form and return it to the Parish Office.

Parishioner Release to St. Jude Catholic Church for use of name, picture, art, written work, voice, verbal statements, portraits (video or still), personal contact information (where provided) and consent to their use by same.

St. Jude Catholic Church agrees that the parishioner's name, picture, art, written work, voice, verbal statements, portraits (video or still), and personal contact information (where provided) shall only be used for public relations, public information, school or parish promotion, publicity, and instruction.  With respect to publication in parish/school web pages, St. Jude Catholic Church and School further agree that:

• Pictures of parishioners of majority age may be published without regard for whether those images contain one, two, or a group of individuals; however, the parish must obtain permission from the parishioner (s) shown in the picture, prior to publishing such images on the parish web site (s). Parishioners of majority age may be identified by first name, last name, or both.
• Committee contact persons must file a signed waiver form with the parish office before personal e-mail addresses and contact phone numbers can be displayed on the parish web site (s). This waiver shall remain on file for a period of not less than 12 months following the last occurrence of this personal information on the parish web site (s).
• E-mail addresses will not be passed on unless requested by the sender. At no time will St. Jude Parish or School share a parishioner’s personal information with third parties. This includes names, e-mail addresses, phone numbers and mailing addresses.
• St. Jude Catholic Church and School will immediately comply with any request  for the removal of specific photographs featuring the parishioner and/or personal contact information for same.

The Parishioner undersigned below understands and agrees that:

• No monetary consideration shall be paid;
• Consent and release have been given without coercion or duress;
• This agreement is binding upon heirs and/or future legal representatives;
• The photo, video or statements may be used in subsequent years.
